The identification of safety critical tasks must be followed by task and human failure analyses in order to gain a thorough understanding of the tasks and identify safety-critical steps. These safety-critical steps must then be subjected to in-depth analysis.
The goals of the in-depth analysis are the systematic identification of the different types of human failures (slips, lapses, mistakes, violations, etc.), and the evaluation of performance influencing factors.
At KK Risk, our approach to conducting task and human failure analyses is to combine hierarchical task analysis (HTA) with a human-HAZOP. The human-HAZOP methodology is specifically developed with active involvement from front line personnel who currently perform the task being analysed. We use HTA to breakdown a safety-critical task into a sequence of steps so that the safety-critical steps can be identified. The human-HAZOP methodology is then applied to the safety-critical steps to determine if additional safeguards are required.
We can provide training and mentoring to allow site personnel to carry out future assessments independently.
